Troop Policies and Forms
Our Troop uses the following policies and forms:

Medical Form -- Parts “A” and “C” medical form is required BEFORE the scout may participate in any overnight camping activities or BSA events. Summer camp requires section “B” to be completed. This form is valid for one year.

Medical Permission to Render Treatment -- Parents are required to complete a medical release/permission to treat form BEFORE your son can participate in any activity with the troop. The medical form provides leaders with critical information and permission to seek medical treatment. This form must be notarized.

Troop Conduct and Discipline Guidelines -- It is important for the Scout (and his family) to understand that acceptable standards of behavior will be required while they are part of troop 613. This form must be signed by the Scout and his parent/guardian.
- All Scouts must have submitted a "Permission Slip" before going on any outing with our Troop. (Link to a generic Permission Slip.)
- The procedure for getting non-Scout related service projects approved in advance by a Scoutmaster is to complete this Community Service Report Form.
- All adults volunteering with the Troop must follow the Vounteer Rules information.
- All adults camping with the Troop must follow the Adult Participation information. (This includes adult registration through the Troop.)
- To fulfill First Class requirement 11, recruiting a new Scout, Scouts can complete the Recruiting Report Form.
